Salary Range: $3,293.00 – $3,640.00 monthly Work Schedule:

SUMMARY Antelope Valley Partners for Health (AVPH) is a
community-focused organization dedicated to improving the health,
wellness, and quality of life of individuals and families
throughout the Antelope Valley. We partner with local agencies,
funders, and community stakeholders to deliver programs that
promote health equity and strengthen community resilience. JOB
SUMMARY The Community Health Worker (CHW) plays a key role in the
Community Public Health Teams (CPHT) initiative—a collaborative,
place-based public health model serving high-priority micro-service
areas across Los Angeles County. This field-based role is
responsible for direct outreach, household engagement, education,
service connection, and data collection for 8,000–13,000 households
within assigned census tracts. CHWs support community action, build
trust with residents, and help address health disparities by
connecting households to essential public health resources. KEY
RESPONSIBILITIES Outreach & Community Engagement Conduct
door-to-door canvassing and annual household visits using the CPHT
buddy-system safety protocol. Engage directly with residents
through outdoor outreach in neighborhoods, community centers,
schools, and events. Build trusting relationships with households
and provide in-person education, resource information, and service
linkage. Support on-site health screenings, workshops, and
community-based events. Service Navigation & Referral Support
Connect community members to appropriate health and social
services. Conduct Household Assessments and document identified
needs, barriers, and follow-up actions. Maintain timely and
accurate documentation of referrals, household interactions, and
service outcomes. Data Collection & Reporting Enter outreach and
household data using mobile and web-based tools. Maintain
confidential records in accordance with AVPH and funder
requirements. Track and report progress toward deliverables,
including household visits and linkage metrics. Program Support &
Public Health Response Participate in community convenings, action
plan efforts, and neighborhood coalitions. Assist in public health
response activities, such as vaccine outreach or disease
investigation, as requested by DPH. Fulfill other program-related
duties as assigned. TRAVEL REQUIREMENTS This is a field-based role
requiring: Daily travel by foot and vehicle within Antelope Valley
communities (e.g., East Lancaster, Central Lancaster). Reliable
transportation, valid CA driver’s license, and current auto
insurance. Travel to partner meetings, trainings, and LACDPH sites
as scheduled. Mileage is reimbursed according to AVPH policy.
REQUIRED QUALIFICATIONS High school diploma or equivalent required;
bachelor’s degree in public health, social work, or related field
preferred. Minimum 3–5 years of experience as a CHW, promotora,
outreach worker, or similar role. Experience working with
underserved communities and familiarity with local resources.
Ability to meet health and background requirements (TB test,
COVID-19 vaccine card, LiveScan). CA driver’s license and reliable
transportation required. PREFERRED QUALIFICATIONS Bilingual
English/Spanish highly preferred (strong preference for
reading/writing in Spanish). Experience working in high-need or
high-risk communities. Knowledge of Los Angeles County public
health systems, community programs, and local resources. KNOWLEDGE,
SKILLS & ABILITIES Strong interpersonal communication skills;
ability to build rapport quickly. Demonstrated ability to work
independently in community settings with minimal supervision.
Ability to walk 1–3 miles per day and navigate varied outdoor
environments. Culturally sensitive, non-judgmental, and able to
engage diverse populations. Skilled in decision-making, time
management, and prioritizing multiple tasks. Proficiency with Word,
Excel, Outlook, and mobile data collection tools. Commitment to
confidentiality, professionalism, and safety protocols. PHYSICAL
REQUIREMENTS & WORKING CONDITIONS Field-based role with daily
walking, standing, outdoor exposure, and travel between sites.
Regular exposure to varied weather and high-activity environments.
Ability to lift/carry up to 25 lbs., stoop, bend, squat, and
occasionally run if needed for safety. Occasional office-based
meetings or required trainings.
