Growing together. This position is full-time, Monday – Friday.
Employees are required to work our normal business hours of 8:00am
– 5:00pm PST. It may be necessary, given the business need, to work
occasional overtime. Our office is located at 5750 Downey Avenue,
Lakewood, CA, 90712. This will be on-the-job training. The hours
during training will be 8:00am – 5:00pm PST, Monday – Friday.
Primary Responsibilities: Job Responsibilities include but not
limited to: - Coordination of discharge plan for patients being
discharged from hospitals/facilities. This is working on site
position. - Coordination with hospital and Optum CM to
facilitate/expedite patient discharge in a timely and/appropriate
manner. - Arrangement of skilled nursing facility bed (SNF) for
patient discharge to SNF. - Arrangement of home health, durable
medical equipment-oxygen, walkers, wound supplies, feeding supplies
and other DME related needs. - Arrangement and authorization of
ambulance/ other transportation needed. - Arrangement of dialysis
chair time. - Co-ordination of all post discharge appointments
include post discharge visit with PCP and specialist appointment. -
Communication of discharge plan to patients, families and/hospital
CM’s. - Entry of all authorizations and documentation in CUR/other
platforms as needed. - Other duties as assigned. You’ll be rewarded

you may be interested in. Required Qualifications: - High School
Diploma / GED OR equivalent years of experience - Must be 18 years
of age OR Older - 3 years of experience in Healthcare setting
(doctor’s office or acute care setting) - 1 years of experience
with Medical terminology, CPT and ICD-10 coding - Experience with
computers and Windows based programs including the ability to work
with multiple computer platforms simultaneously - Ability to work
our normal business hours of 8:00am – 5:00pm PST. It may be
necessary, given the business need, to work occasional overtime
Preferred Qualifications: - Basic understanding of managed care
principles Soft Skills: - Excellent communication verbal and
written - Excellent organization and customer service skills -
Detail-oriented The hourly range for this role is $19.86 to $38.85
per hour based on full-time employment. Pay is based on several
factors including but not limited to local labor markets,
