Job Description:
A company is looking for a Clinical Navigation, Family Nurse
Practitioner (Per Diem).
Key Responsibilities:
Assess patient needs, gather clinical history, and provide guidance
to navigate healthcare resources
Deliver high-quality clinical services and monitor care quality to
meet patient satisfaction benchmarks
Evaluate and manage patients with acute, chronic, and mental health
conditions while collaborating on efficiency improvement
projects
Required Qualifications:
Licensed NP/PA provider in good standing with state board
Maintains board certification
At least 3 years of NP/PA experience in a clinical setting
Comfortable providing care to all age groups
Must be in PST or MST time zone
